Custom Intake Form

Studios can now customize the intake questions clients see during account creation. With Custom Intake Forms, studios have more control over what information is collected during the client sign-up flow.

This update enhances the existing Additional Details section in the sign-up flow. All collected details will appear in the Front Desk and Widget Client Profiles.

Why It Matters:

Every studio is different. Until now, all studios used the same fixed intake questions, which meant:

  • Important health or onboarding details were missed

  • Staff weren’t fully prepared for a client’s first visit

  • Studios relied on manual workarounds

  • Some questions felt unnecessary or misaligned with studio values

Functionality:

A new Client Intake Questions section is available in Business Settings, where studios can manage intake questions for their entire organization.

Standard Intake Questions

(Standard questions that already existed in the system)

  • Studios can turn standard questions on or off

  • Studios cannot edit standard questions

  • If a standard question is turned off:

    • It will not appear in the client sign-up flow

    • Past answers remain visible in the Front Desk client profile

  • Turning off Birthday or Referral Source may affect automations (there is a tooltip next to these to reflect that this impacts automations)

Ability to Add Custom Questions

Studios can add their own custom intake questions to collect additional details.

  • Additional questions are free-form text (question + free form answer)

  • Custom questions appear immediately in the new client sign-up flow

  • They show up in both the Front Desk and Widget client profiles

  • If a client already completed intake, new questions will appear as blank in front desk and widget client profiles

  • Editing a question updates it going forward

    • This means if a studio edits an existing question and completely changes the question type. All previously answered questions will still remain and may not make sense.

    • Recommendation: Keep the original essence of the question. Do not change completely. If total changes are needed, the studio should make question inactive and create new one.

  • Removing a question:

    • Past answers remain visible in the front desk client profile

    • The question and answer will no longer be visible in the widget client profile

    • The question is removed from the intake form moving forward

Client Profile (Front Desk)

  • All intake responses continue to live in the More Info section

  • Standard questions appear first

  • Custom questions appear below standard questions

  • Removed standard questions remain visible but blank if unanswered

  • Staff can manually add or update responses using the Edit button
